Chuck Forman

Chuck Forman, 52, of Middleton, passed from this life on Tuesday, December 26, 2023, at Bolivar General Hospital.

He was born Sunday, September 19, 1971, in Selmer, and lived his life here.  He worked since the age of 18 at Thyssen-Krupp and was currently a machine operator.  He enjoyed working, and also hunting duck and deer.  Most of all, he loved doing whatever his son, Drew, wanted to do.  He was an avid animal lover, especially with his own special dog.  He married Mandy Cox on February 14, 1994, and together they enjoyed the quiet life, spending time at their home and doing family things.  Mr. Forman attended Pocahontas First Baptist Church.

Survivors include his wife, Mandy; his son, Drew Forman of Middleton; his mother and father, Jerry and Glenda Smith of Eastview; two sisters, Angie Browder (Robert) of Pocahontas and Callie Allison (Mark) of Selmer; a brother, Dusty Smith of Eastview; two nieces, BreAnna Khoury (Wissam) and Maddie Browder; a great niece, Ava; a great nephew, Zeke and a great nephew "on the way".  He was preceded in death by his grandparents, Nina Mae Jenkins and Hubert Lee Jones.  Memorial contributions in his name may be sent to St. Jude Children's Research Hospital (www.stjude.org/donate).
